| PHCS: Physician & Healthcare Facility Network |
 Nu-Health-Network provides its members access to the PHCS network. Founded in 1999, PHCS is one of the largest and fastest-growing preferred provider organizations in the country, encompassing more than 400,000 physician providers and 5,500 healthcare facilities.
PHCS' mission is to design and build the "next generation" of PPO services. This is extremely important in an industry where networks have been the same for a quarter of a century, and at a time when healthcare cost is escalating rapidly.

| Pharmacy Network |
 After receiving a prescription from their physician, Nu-Health-Network members simply take the prescription, along with their Membership ID Card, to one of the more than 50,000 EHO Nationwide Network Pharmacies. The Membership ID Card contains the necessary information the pharmacist will need to submit the claim to EHO. Whether a member requires brand-name or generic drugs, Nu-Health-Network's pharmacy network will help member save up to 40% on their prescription needs.

| Dental Care |
 Nu-Health-Network's Careington dental program will save members money on normal procedures such as exams, x-rays, dentures, and fillings, as well as some specialty services including orthodontics, periodontics, root canals, etc. Cosmetic procedures are not normally covered under this program. A copy of the dental fee schedule for members is included in the Provider Directory.

| Chiropractic Care |
 This network entitles Nu-Health-Network members to a 30% discount off of the usual and customary fees from the list of chiropractors found in the Provider Directory. There is no charge for the first consultation with these chiropractors. In addition, all x-rays and other diagnostic tests will be offered at a 50% discount (except in the state of Colorado).

| Alternative Healthcare |
 Health Solutions includes access to savings on alternative providers. The services include savings on massage therapy, acupuncture, aromatherapy, ayuerveda, biofeedback, chelation therapy, kinesiology, nutrition products and counseling, Qi Gong, Tai Chi, Reki, yoga, and numerous other specialties. An alternative provider accessed through the Health Solutions network generally provides a 20% savings for Nu-Health-Network members.

| Long Term Care |
 Participating in the Long-Term Care Network assures members' access to high-quality medical providers at a reduced price. Through an arrangement with HealthCare Synergies, Inc. (HSI), members may be eligible to receive substantial discounts from more than 3,000 providers of long-term care services. The network includes the following types of providers and services: |

Nu-Health-Network members may be able to save from 5% to 30% (national average is 15%) off the usual and customary fees for services from HSI Network Providers. There is no length of stay requirement to receive these contracted rates.

| Eye Care Savings |
 Savings on prescription lenses with Eye Care International (ECI) will range from 20% to 50% for Nu-Health-Network members. Savings on eye exams are available to you in select limited areas of the country. Members should check their Provider Directory to see if there is a location near them that offers discounts on eye exams. The Eye Care International Network also includes a mail order contact lens replacement program that offers up to 60% savings on lenses and solutions.
The Lasik City offers Nu-Health-Network members and their families the opportunity to have the Lasik Vision Correction procedure at substantial savings to Nu-Health-Network members.

| Hearing Aid Savings |
 The Nu-Health-Network programs allow members the opportunity to purchase over 80 models of major brand-name hearing aids at up to 15% below retail dealer prices through BelTone Hearing Services. This excellent pricing schedule includes a 100% refund guarantee if the member is not completely satisfied with the hearing aid. Members can also save on repair services and hearing aid batteries through this program.
